Publication Early postoperative outcomes for pancreaticoduodenectomy before and after implementation of enhanced recovery after surgery (ERAS) protocol(Surgical Society of Northern Greece, 2017) ;Ling Q.Y. ;Ariffin A.C. ;Azman A. ;Zuhdi Z. ;Othman H.Jarmin R.Background: We implemented the Enhanced Recovery After Surgery (ERAS) for our post pancreaticoduodenectomy patients in our institution since 2013. This study aim to to determine the differences between the previous postoperative care and ERAS. Material and Method: We included all patients who underwent pancreaticoduodenectomy UKM Medical Centre (UKMMC) from Jan 2011 to April 2015. Subjects are divided into control group comprise pre-ERAS patient and study group comprise post ERAS implementation. Recorded data include demographics, patients� characteristic, surgery data, complication, outcomes and length of stay. Results: Total subjects were 81 patients. There were no significant differences in the demographics and patients� characteristic between both groups. Post-operative morbidity, re-laparotomy and readmission rate was equivalent. Mortality rate were more in pre-ERAS group but not significant. The length of stay was lower in the post ERAS group (11.7 days versus 15.4 days; p=0.002). Non-PD related complication is high within the pre-ERAS group (27.7% versus 5.9%; p=0,019). Conclusions: ERAS protocol have a lower complication rate and reduce hospital stay. The presentation is usually non-specific requiring a high index of suspicion in its diagnosis. Although this condition is considered benign, the implication of an untreated biloma is severe. Case presentation: We presented a case of a 25-year-old man who is involved in a road traffic accident and sustained multiple liver lacerations. Liver packing was done with no immediate complication. However, he developed an infected biloma during recovery. Magnetic resonance cholangiopancreatography confirmed a transected right hepatic duct. Biliary duct stenting and percutaneous was done successfully. Conclusion: Biloma is common in high-grade liver injury. Publication Improvement of endoclip design using 3D CAD approach(Institute of Physics Publishing, 2020) ;Siregar I. ;Izham M.I.C. ;Ariffin A.C. ;Adenan M.S.Saedon J.B.Endoscopic clip (endoclip) is a metallic mechanical device used in gastrointestinal endoscopy practices with the intention of close mucosal surfaces deprived of the requirement for surgery and sew up. Nowadays, endoclip is accessible in the market predominantly in single firing mode, where the clip can only be used once in a period of process. Due to that, the clip is loaded to its applicator repeatedly during the operation which cause delay for the surgery that can lead to fatality. Therefore, this project aims to develop a multi-firing endoclip cartridge that can reduce the difficulty of reloading the endoclip system to its applicator by improving current design. Three design of multi-firing endoclip cartridges were produced by using different mechanism to apply the endoclip. The casing of the cartridge was designed in cylindrical shape with 40 mm diameter and 68 mm length with CATIA V5 software. These three designs formerly assessed by using decision matrix method for assortment of the best concept design. The result from the decision matrix table shows that design with revolve internal clip holder got the highest decision matrix selection amongst the three designs, advantaging in holding more clips and can reduce operation time compared to the other two designs. � Published under licence by IOP Publishing Ltd. - Some of the metrics are blocked by yourconsent settings

It is uncommon sequelae of chronic pancreatitis. Smith first discovered it in 1953 in the literature. The etiology includes chronic pancreatitis, pancreatic pseudocyst, pancreatic trauma and idiopathic in origin. Despite a rise in the incidence, it is scarcely reported. This is a case of a 20-year-old man presented with repeated acute pancreatitis episode complicated with gross ascites. Pancreatic duct stones were identified intra-operatively. It is however, not without complications. Portal vein thrombosis following RFA is rare but can be fatal to the outcome of the patient. Here, we present a case of a 66-year-old man who developed portal vein thrombosis following RFA. CT scan revealed a left portal vein thrombosis. In essence, endoclip is a mechanical device in the world of health, especially for endoscopic activities, which are also used for the process of hemostasis and also non-hemostatic applications. This study describes the relationship between end-time end-endoscopic problems during gastrointestinal standards. Currently, market-available endoclips only focus on single-firing, where the process is carried out one by one. The use of Endoclip at this time is very difficult because of the repetition of the clip loading process to the applicator. Therefore, the purpose of this research is to design cartridges which consist of several clips for endoclip and allow for mechanical movements. The method of selecting the appropriate and important part of the tip which is the most important way to solve the problem and the design and process of the simulation is done using CATIA V5 software. Publication Novel usage of fresh frozen plasma in hereditary angioedema(Societa Editrice Universo, 2016) ;Ngadiron H. ;Ariffin A.C. ;Adwa F.N. ;Ali S.M. ;Isa R.M. ;Faculty of Medicine and Health Sciences ;Universiti Sains Islam Malaysia (USIM)Hospital AmpangHereditary angioedema (HAE) is a rare and potentially life threatening autosomal dominant disease characterized by recurrent episodes of cutaneous and mucosal oedema. It results from reduced expression or loss of function of CI-esterase inhibitors (C1-INH). As opposed to the more common histamine-mediated angioedema, HAE does not respond well to conventional treatments with anti-histamines, steroids and adrenaline. Early recognition and timely intervention with the correct treatment are crucial particularly preventing airway obstruction. New disease specific treatment including plasma derived or recombinant C1-INH, ecallantide and icatibant have recently emerged and its appropriate use can reduce HAE-associated mortality and morbidity. However due to its costs, these disease specific treatments have yet to reach Malaysia. Despite that no randomized clinical trial on FFP has been performed, its efficacy in treating acute attacks of HAE is only demonstrated in case studies. No consensus or guidelines about the emergency management of long-term complications following bariatric surgery are currently available. The aim of this study is to investigate by a web survey how an emergency surgeon approaches this unique group of patients in an emergency medical scenario and to report their personal experience. Method: An international web survey was sent to 197 emergency surgeons with the aim to collect data about emergency surgeons' experience in the management of patients admitted in the emergency department for acute abdominal pain after bariatric surgery. The survey was conceived as a questionnaire composed by 26 (multiple choice and open) questions and approved by a steering committee. Results: One hundred seventeen international emergency surgeons decided to join the project and answered to the web survey with a response rate of 59.39%. Conclusions: The aim of this WSES web survey was to highlight the current management of patients previously submitted to bariatric surgical procedures by ES. Emergency surgeons must be mindful of postoperative bariatric surgery complications. CT scan with oral intestinal opacification may be useful in making a diagnosis if carefully interpreted by the radiologist and the surgeon. In case of inconclusive clinical and radiological findings, when symptoms fail to improve, surgical exploration for bariatric patients presenting acute abdominal pain, by laparoscopy if expertise is available, is mandatory in the first 12-24 h, to have good outcomes and decrease morbidity rate. � The Author(s). 2020. - Some of the metrics are blocked by yourconsent settings

Prophet Muhammad (PBUH) had taught us about Solat during sickness. As narrated by ‘lmran bin Husain had piles so I asked the Prophet about the prayer. He said “Pray while standing and if you can’t pray while sitting and if you cannot do even that then pray lying on your side.” Having the knowledge of performing Solat during sickness is crucial in allowing Muslim patients to perform Solat despite being hospitalized. This study aims to assess the level of knowledge, attitude and practice of Solat and ablution among hospitalized Muslim patients and its association with the demographic data. This is a cross sectional study among hospitalized Muslim patients at selected hospitals in Malaysia caring for medical, surgical, orthopedic and otorhinolaryngology patients. Patients are required to answer a set of questions carry out by face-to-face interview. The response rate for the study sample was 95.8%. Out of total 385 respondents, 255 (66.2%) were male and 130 (33.8%) females. Mean age of the respondents was 46.89±16.77 years. 45% respondents have at least completed secondary education. 21% (n = 81) respondents know about rukhsah solat. Only 8% respondents received guidance of rukhsah solat that are mostly from the family members. From all the respondents only 67% perform daily five times solat at home and 48% did not perform solat during hospitalization. The mean knowledge score is 17 (max 22), mean attitude score is 41 (max 55) and mean attitude score is 9 (max 12). Knowledge, attitude and practice score are moderate among patients. - Some of the metrics are blocked by yourconsent settings
