Publication Myelin Oligodendrocyte Glycoprotein (MOG) Optic Neuritis: A Case Series(Cureus, 2021) ;Masnon Nurul-Ain ;Zuhratun Nazihah Khairul Kamal ;Wan-Hazabbah Wan Hitam ;Maimunah Abdul Muna'aimFaizah Mohd ZakiMyelin oligodendrocyte glycoprotein (MOG) antibody disease has been recognised as a distinct demyelinating disorder. Optic neuritis has been reported as the most common presentation and manifestation of this spectrum disorder. This is a case series of three MOG optic neuritis patients. Patients involved are female with disease onset ranging between 7- and 37-year-old. Most of these patients experienced symptoms of profound reduced visual acuity with eye pain. All three patients had optic disc swelling upon first presentation and they experienced at least one episode of bilateral simultaneous optic neuritis. Only one patient had demonstrable optic nerve enhancement on magnetic resonance imaging (MRI). Disease was confirmed through positive MOG antibody. Patients typically responded well to intravenous methylprednisolone (IVMP) during acute attack of optic neuritis. However, one patient had suboptimal response to IVMP after multiple relapses. We noted multiple relapses of optic neuritis are common in MOG patients. MOG optic neuritis is a devastating, but treatable condition. Aggressive treatment during acute optic neuritis attack and relapse prevention may favour a good visual prognosis in MOG antibody disease. - Some of the metrics are blocked by yourconsent settings
Publication Open Globe Injury in Hospital Serdang - An 8-year Retrospective Review(Universiti Putra Malaysia (UPM), 2017) ;Nazri Omar ;Maimunah Abdul Muna'aim ;Rozita IsmailRafidah Md. SalehIntroduction: Open globe injury may lead to devastating visual outcome. Urgent management and subsequent follow-up is important to maximise recovery. More data on open globe injury in Malaysia is needed to identify risk groups and prognostic factors. This study was carried out to investigate open globe injury, the socio-epidemic profile, clinical characteristics and outcome of the open globe injury in the Hospital Serdang, Malaysia. Methods: All patients managed for open globe injury to Hospital Serdang from January 2006 to December 2013 were included in this retrospective case review. Student's T-test was used to determine difference between means and chi-square for categorical data. P value of less than 0.05 was regarded as statistically significant. Results: There were 155 patients managed for open globe injury with three of them had bilateral involvement. As such, there were 158 cases eligible for analysis in this study. The average age was 32.2 ± 16.5 years (mean ± standard deviation, SD). Male was at higher risk for open globe injury. Predictors for poor visual outcome were foreign nationality (p=0.047), lid laceration (0.008), type of injury (p=0.001), site of injury (p=0.008), RAPD (p<0.001), uveal prolapse (p<0.001), hyphaema (p=0.008), lens damage (p=0.010), vitreous loss (0.014), retinal detachment (p=0.011), intraocular foreign body (IOFB) (0.014) and poor presenting uncorrected visual acuity (UCVA) (p<0.001). Conclusions: Occupational injury was a main cause of open globe injury in Hospital Serdang. Although in general the visual outcome is good, effort should be put to prevent work-related injury. - Some of the metrics are blocked by yourconsent settings
Publication Surgical Instrument’s Tip Fracture During Cataract Surgery(USIM Press, 2021) ;Azlina Mokhtar ;Zalifa Zakiah Asnir ;Maimunah Abdul Muna'aimUmi Kalthum Md NohPurpose: To describe a case of a surgical instrument’s tip fracture during cataract surgery. Method: Case report. Results: We report a case of a 60-year-old gentleman who underwent phacoemulsification of the left eye. It was noted that the tip of the second instrument (lens chopper) was fractured during the last quadrant removal of the opacified lens, which disappeared posteriorly. The surgery was successfully completed, and the intraocular lens appeared stable without posterior capsular rent. An urgent computed tomography scan (CT scan) of orbit was arranged. It showed a round radio opaque foreign body near the lens at the lateral aspect of the left intraocular lens (IOL), measuring approximately 0.3cm. The patient underwent foreign body removal using an intraocular magnet by a vitreoretinal surgeon two days after the phacoemulsification surgery. The patient had an uneventful recovery with a vision of 6/60 pinhole 6/6 at six weeks post operatively. Conclusion: Surgical instrument’s tip fracture is a known complication during phacoemulsification; however, it was under-reported among surgeons. Careful inspection of all instruments before introducing them to the eye is mandatory, and early identification of this condition during surgery may avoid major complications.
