Publication A non-surgical management of delayed diagnosis of an oesophageal perforation due to ingestion of okra: A case report(Universiti Putra Malaysia Press, 2020) ;Ismail R. ;Mahamad U.A.Marwan A.A.Oesophageal perforation is a not uncommon condition, yet it carries a high mortality rate and has been observed as the most grievous trauma to the digestive tract. Common causes include iatrogenic instrumentation, foreign-body swallowing, and physical injury. This report highlighted a case of oesophageal perforation complicated by formation of proximal descending aorta pseudoaneurysm as a result of okra ingestion. The patient was successfully treated with conservative treatment. The possible mechanism of oesophageal rupture, diagnosis, treatment, and other complication will be further discussed. � 2020 UPM Press. Publication Knowledge, attitude, practice and stigma of tuberculosis among healthcare providers in hospital ampang(Universiti Putra Malaysia Press, 2020) ;Shahbuddin H.M.A.-W. ;Mahamad U.A.Marwan A.A.Introduction: Malaysia is an intermediate tuberculosis burdened country as classified by WHO. There was a recent rise in TB incidence. Healthcare providers, especially in secondary or tertiary centers, were constantly exposed to TB infected patients, which is a hazard source for TB infection. Thus, assessment of behavior towards TB is paramount through knowledge, attitude, health-seeking practice, and stigma domains. Method: A cross-sectional survey was carried out between November 2014 and February 2015 among healthcare providers in Medical Department, Hospital Ampang. Data were analyzed using SPSS ver 21. Results: Despite overall good knowledge on TB, only 20% of the correspondence knew which patient's population is at risk of TB infection. Almost all perceived TB as a health threat and willing to seek medical care if they developed TB symptoms. However, a small proportion (5.3%) will keep it a secret if they contracted TB. This is contrasted by the majority that perceived TB patients will not be accepted by the community. Conclusion: Knowledge and attitude towards TB was adequate within the healthcare providers in Medical Department, Hospital Ampang despite the presence of some knowledge gaps. However, there was high perceived stigma that needed to be addressed in order for them to deliver the best medical care. � 2020 UPM Press. Publication Necrotizing autoimmune myopathy: A case series(Universiti Putra Malaysia Press, 2020) ;Sahari N.S. ;Marwan A.A. ;Suahilai D.M. ;Jamid N.M.Shahril N.S.Necrotizing autoimmune myopathy (NAM) is considered a new subgroup of a rare autoimmune idiopathic inflammatory myopathies. Classically, NAM presented with sub-acute onset of proximal muscle loss of power with raised creatinine kinase and characteristic muscle biopsy showing muscle necrosis and regeneration with little inflammation. Statin use, connective tissue diseases, malignancy and HIV infection are the identified risk factors for NAM. The autoantibodies expected to be presented in NAM are anti-signal recognition particle (SRP) and anti-hydroxymethylglutaryl-coenzyme A reductase (anti-HMGCR) antibodies. In this article, we present three cases of NAM with different risk factors and autoantibodies which we believe to have impact on the clinical course and outcome of our patients. � 2020 UPM Press. Publication Posterior reversible encephalopathy syndrome in systemic lupus erythematosus: Pooled analysis of the literature reviews and report of six new cases(2013) ;Shaharir S.S. ;Remli R. ;Marwan A.A. ;Said M.S.M. ;Kong N.C.T. ;Faculty of Medicine and Health Sciences ;Universiti Kebangsaan Malaysia (UKM) Medical CentreUniversiti Sains Islam Malaysia (USIM)Introduction: Posterior reversible encephalopathy syndrome (PRES) is a rare neurological disorder which is increasingly recognized to occur in systemic lupus erythematosus (SLE). Objective: The purpose of this study was to identify the characteristics of SLE patients with PRES and the associated factors of the poor outcome among them. Methods: We investigated SLE patients who developed PRES between 2005-2011 at the Universiti Kebangsaan Malaysia Medical Centre. A comprehensive literature search was done to find all published cases of PRES in SLE. Pooled analysis was conducted to identify the factors associated with poor outcome. Results: There were 103 cases of PRES in SLE published in the literature but only 87 cases were included in the analysis in view of incomplete individual data in the remaining cases. The majority of the cases were Asians (74.2%), female (95.4%) with mean age of 26.3±8.8 years. PRES was highly associated with active disease (97.5%), hypertension (91.7%) and renal involvement (85.1%). We found that 79 patients had a full recovery (90.8%) with a mean onset of full clinical recovery in 5.6±4.1 days. On univariate analysis and logistic regression analysis the predictors of poor outcome, defined as incomplete clinical recovery or death, were intracranial hemorrhage, odds ratio (OR) 14 (1.1-187.2), p=0.04 and brainstem involvement in PRES, OR 10.9 (1.3-90.6), p=0.003. Conclusion: Intracranial hemorrhage and brainstem involvement were the two important predictors of poor outcome of PRES. Larger prospective studies are needed to further delineate the risk of poor outcome among them.
