Publication Colorectal Cancer Patients In A Tertiary Referral Centre In Malaysia: A Five Year Follow-up Review(Asian Pacific Journal of Cancer Prevention, 2009) ;Mohd Radzniwan A Rashid ;Aznida Firzah Abdul Aziz ;Saharuddin Ahmad ;Shamsul Azhar ShahIsmail SagapBackground: Colorectal cancer (CRC) is one of the major malignancies in the world. In Malaysia, CRC isfast becoming the commonest cause of cancer death. Its etiology is complex, involving both environmental andgenetic factors. This study looked at the profile and outcome of five-year follow-up of patients with CRC. Materials and Methods: Retrospective case review study done on CRC patients at Universiti Kebangsaan Malaysia MedicalCentre (UKMMC), Kuala Lumpur, Malaysia. Patients’ socio-demographic characteristics, modalities oftreatment, cancer characteristics and outcome at 5-year follow up were extracted from the case records. Results:A total of 107 case records of patients were analyzed. Peak age of CRC presentation was 40-69 years (71.1%).Male to female ratio was 1.2:1 with Chinese predominance (52.3%). Anaemia and its related symptoms includingper rectal bleeding was the commonest clinical presentation. The median duration of clinical presentation was13 weeks (IQR 21.8). More than two-thirds presented as non-emergency cases (69.2%). Most patients presentedwith Dukes C stage(40.2%). The overall 5-year survival rate was 40% with local recurrence rate of 19.6%.Metastasis after curative-intend treatment (surgery with adjuvant therapy) developed in 26% of patients. Lowerrecurrence (p = 0.016, OR = 0.205) and metastatic disease (p = 0.02, OR = 0.24) found among the Chinesepatients. Almost half of the patients defaulted follow up care (43%), most often within the first year of treatment(22.4%) and the Chinese were the least likely to default (p= 0.04, OR = 0.45). Conclusion: Socio-demographicprofile of CRC patients in UKMMC is comparable to Asia pacific region. Publication Determinants of patients at risk of developing obstructive sleep apnea in a primary care clinic(Hilaris Publishing, 2014) ;Mohd Radzniwan A Rashid ;Saharuddin Ahmad ;Aida Jaffar ;Fairuz AliNorliza PaidiIntroduction Obstructive sleep apnea (OSA) is associated with multiple medical and psychosocial impacts. Current evidence on individuals at risk of developing OSA in Malaysia particularly at primary care setting is scarce. This study aimed to determine the magnitude of patients at risk of developing OSA attended an suburban primary care clinic and associated factors in southern Malaysia. Methodology: This was a cross sectional study of adults≥ 18-year old using systematic random sampling. Berlin Questionnaire was used to determine the risk of developing OSA. Respondent’s socio-demographic characteristics and clinical parameters including body mass index, weight, blood pressure, neck and waist circumference were obtained. Data was analyzed using SPSS version 20.0. Results: Out of 160 participants, 65.6% were females with the median (IQR) age of 55.6 (27.75) years and 81.9% were obese. Self-reported hypertension and diabetes was 55.6% and 35.60% respectively. One-fifths (19.4%) had high risk of developing OSA. Regarding symptoms described in the Berlin questionnaire, 37.5% reported snoring and 10.6% had excessive daytime sleepiness. The risk of developing OSA is associated with gender (p= 0.008), neck circumference (p= 0.010) and diastolic blood pressure (p= 0.015). Multivariate logistic regression found the only factor significantly associated with at high risk of developing OSA is diastolic blood pressure (p= 0.031). - Some of the metrics are blocked by yourconsent settings

Now not only ‘SMS’, even various other forms of online communication arise such as such as WhatsApp, Telegram, Messenger WeChat, Instagram and many others. In addition to providing convenience in the relationship even though the distance is thousands of miles, 'SMS' also has an impact on the law in certain cases. Among the questions that arise is the acceptance of hibah made by ‘SMS’ for those suffering from COVID-19 at the level of Maraḍ al-Mawt according to Islamic law. This article explored the concept of ‘SMS’ as an electronic document and its power in proof in a grant-related case by a COVID-19 patient in a Maraḍ al-Mawt situation. In addition to tracing the concept of Maraḍ al-Mawt and its conditions according to fiqh and legislation, this article will also link it to COVID-19 and ‘Terminal Disease’ according to modern medicine. Kaedah kualitatif ini bagi menjawab beberapa persoalan dan objektif iaitu konsep dokumen elektronik dalam pembuktian, penerimaan hibah dalam situasi maradh al-mawt bagi pesakit COVID-19 dan sejauh manakah COVID-19 mempunyai hubungan kait dengan mardh al-mawt. Publication Knowledge Of Colorectal Cancer Among Patients Presenting With Rectal Bleeding And Its Association With Delay In Seeking Medical Advice(Asian Pacific Organization for Cancer Prevention, 2011) ;Syahnaz Mohd Hashim ;Tong Seng Fah ;Khairani Omar ;Mohd Radzniwan A Rashid ;Shamsul Azhar ShahIsmail SagapBackground: Knowledge is believed to be a driving factor for patients’ early presentation for healthcare. This study was conducted to assess knowledge of colorectal cancer among subjects presenting with rectal bleeding and to determine its association with late presentation. Methods: A cross-sectional study of 80 patients with rectal bleeding, aged 40 and above, was conducted between December 2008 and June 2009 in the endoscopy unit, University Kebangsaan Malaysia Medical Centre. The research instruments used in this study was a self-administered questionnaire including data on duration of rectal bleeding, first medical consultation and knowledge of colorectal cancer. Results: Sixty percent of subjects with rectal bleeding delayed seeking medical advice. Subjects were more aware of symptoms of non-colorectal cancers compared to symptoms of colorectal cancer. The majority of subjects (63.8%) correctly identified rectal bleeding as a symptom but were not aware of the best screening method to detect colorectal cancer. Half of the subjects knew increasing age and genetic background to be risk factors for colorectal carcinoma. However, knowledge of colorectal cancer was not found to be significantly associated with delay in seeking help. Conclusion: Findings indicate poor awareness of colorectal cancer among the subjects. Although public education of colorectal cancer is important for early presentation on rectal bleeding, further studies are advocated to evaluate other factors influencing patients’ help seeking behavior other than knowledge. Increasing demand of daycare centres showed ramiϑication on unintentional injuries at the childcare settings. The incidence is becoming more common and this may lead to a lawsuit for negligence. Failure to provide proper ϑirst aid response is one of the issues faced by the caregivers. To the best of our knowledge, this is the ϑirst study investigating the effects of ϑirst aid training and certiϑication on knowledge, attitude and conϑidence (KAC) among childcare providers in Negeri Sembilan, Malaysia. A self-administered questionnaire was given to 54 Negeri Sembilan childcare providers before and after they received a comprehensive module on the ϑirst aid training. The training comprised of three major components; basic life support, trauma injuries and non-trauma injuries. Throughout the training, participants were given a series of lectures, hands-on practicals and small group discussion on speciϑied topics from the module. Overall results showed a signiϑicant increase in knowledge, attitude and conϑidence (p<0.001) after the respondents’ had received the FAT4CP© training. The ϑirst aid training provided to the childcare providers was found effective and had successfully improved their knowledge by 12.5% and their conϑidence to perform ϑirst aid increased by 20.0%. They are now more prepared and efϑicient at handling any emergency situations at childcare centres. Our results suggested that FAT4CP© training programme was able to increase knowledge, attitude and conϑidence of childcare providers in Negeri Sembilan to handle emergency cases. Therefore, the programme is suggested to be implemented at all childcare centres in order to improve and sustain the providers’ abilities in handling emergency situations. - Some of the metrics are blocked by yourconsent settings

Design Cross-sectional study. Setting This study is part of a larger national study on the perception of the Malaysian public healthcare professionals on FMSs. Participants PHCPs from three categories of health facilities, namely hospitals, health clinics and health offices. Main outcome measures Qualitative analysis of written comments of respondents’ expectation of FMSs. Results The participants’ response rate was 58% (780/1345) with an almost equal proportion from each public healthcare facility. We identified 21 subthemes for the 623 expectation comments. The six emerging themes are (1) need for more FMSs, (2) clinical roles and functions of FMSs, (3) administrative roles of FMSs, (4) contribution to community and public health, (5) attributes improvement and (6) research and audits. FMSs were expected to give attention to clinical duty. Delivering this responsibility with competence included having the latest medical knowledge in their own and others’ medical disciplines, practising evidence-based medicine in prehospital and posthospital care, better supervision of staff and doctors under their care, fostering effective teamwork, communicating more often with hospital specialists and making appropriate referral. Expectations ranged from definite and strong for more FMSs at the health clinics to low expectation for FMSs’ involvement in research; to mal-expectation on FMSs’ involvement in community and public health programmes. Conclusions There were some remarkable differences in expectations on FMSs from the three different PHCPs. These ranged from being clinically competent and administratively available for patients and staff at the health clinics, to mal-expectations on FMSs to engage in public health affairs. Possible infective aetiologies are commonly adenovirus, respiratory syncytial virus (RSV), influenza, and parainfluenza virus. A 25-year-old student was diagnosed with this condition in infancy and is dependent on 24-hour oxygen for the last 12 years. He was planning on pursuing a law degree in a local public institution. Due to the rarity of this chronic and debilitating illness, the physician attending his check-up must decide on the factors that could affect his academic survival or performance. However, this was overcome with careful risk-benefit considerations and support from family, peers, and physicians. This is postulated due to the presence of multiple comorbidities and age-related immunological changes like immunosenescence and inflammaging. Immunosenescence causes impaired viral clearance, while inflammaging causes exaggerated response inappropriate to stimuli that triggers life-threatening cytokine storm. Hence, home-isolation prevents infection among the elderly and this limits their physical activity. Sedentary lifestyle eventually worsens pre-existing chronic disease and increases the risk of acquiring a new one. Tai Chi is one of the exercises recommended for the elderly during home-isolation. This literature review aims to provide information regarding the groups suggested for Tai Chi, its benefits, as an exercise to be practiced by elderly to maintain general health during COVID-19 and adverse effects. Tai Chi is for those aged 50 years and above or individuals with certain chronic, degenerative or psychological conditions. The systemic benefits are reduction in blood pressure, improves lipid profile, reduction in bone loss and musculoskeletal improvements; whereas neurologically, it reduces cortical shrinkage and improves depression, anxiety and insomnia. Moreover, it induces immune-regulation, which leads to enhancement of the immune system and inhibition of age-related chronic inflammatory condition, inflammaging. These may be protective against severe COVID-19 illness yet further studies are warranted. The adverse effect includes minor musculoskeletal pain and aches. In conclusion, Tai Chi is an ideal exercise for the elderly to maintain health during this challenging pandemic. Face to face teaching was stopped, and lecturers had to find other methods of teaching. Online teaching was the only allowed method. The methods were synchronous and asynchronous. Lecturers have had to find innovative ways to practice effective teaching methods. Assessments had to also be done online taking important factors into consideration, following steps given by authorities. Publication Usage of Traditional and Complementary Medicine (T & CM): Prevalence, Practice and Perception among Post Stroke Patients Attending Conventional Stroke Rehabilitation in A Teaching Hospital in Malaysia(Malaysian Medical Association, 2015) ;Mohd Fairuz Ali ;Aznida Firzah Abdul Aziz ;Mohd Radzniwan A Rashid ;Zuraidah Che Man ;Amnor Aidiliana Amir ;Lim Yinn Shien ;Nurul Shahida RamliNur Asilah Anez Zainal AbidinIntroduction: The lack of evidence that proves the benefit of traditional and complementary medicines (T & CM) in treating chronic medical conditions does not deter its usage among patients worldwide. Prevalence of usage among post-stroke patients in Malaysia especially is unknown. This study aims to determine the prevalence, practice and perception of T & CM use among stroke survivors attending an outpatient rehabilitation program in a teaching hospital. Methods: A cross-sectional study was conducted among 104 post stroke patients attending an outpatient rehabilitation program. A structured self-administered questionnaire was used to collect data on sociodemographic and clinical profile of patients, as well as types of therapy used and perception on T & CM usage. Descriptive analysis was done, and bivariate analysis was used to determine associations between categorical data. Results: Mean age of patients was 62 years (SD 12.2), 54% were Chinese and 75% of the patients had ischaemic stroke. Mean age of T & CM users was younger compared to non- T & CM users (61 years vs. 66 years, p=0.04). Two-thirds (66%) of patients admitted to concurrent T & CM usage while attending conventional post stroke rehabilitation. Acupuncture (40.4%), massage (40.4%) and traditional Chinese medicine (11.5%) were the most common T & CM used. Positive perception was recorded in terms of ability of T & CM usage to relieve post stroke symptoms (68%), and it was safe to use because it was made from 'natural sources'. Negative perception recorded: T & CM caused significant adverse effects (57.6%) and was not safe to be used in combination with other conventional medicines (62.5%). Conclusions: Concurrent T & CM usage among post-stroke patients attending structured outpatient rehabilitation program is widely practised especially acupuncture, massage and traditional Chinese medicines. The establishment of Universiti Sains Islam Malaysia (USIM) Specialist healthcare clinic was founded by the idea of waqf through healthcare for the less fortunate. With collaboration from many agencies the establishment of the clinic has served community in need to access better health care. The primary aim of waqf clinic establishment is to continually uphold the Islamic aspects in its establishment and management for betterment of health among disadvantaged people.
